Must have a Speech Therapist license and have practiced in the field for a minimum of 2 years

Speech Therapists must have a ST License in good standing

Background Screen Level 2

OSHA certificate

HIV/AIDS Certificate

Alzheimer Certificate

Domestic Violence

HIPPA

Current physical within previous 6 months stating, “free of communicable disease"

Current PPD

Current CPR

Social Security Card

Proof of Residency or Citizenship (Work permit/Alien card) (Voter registration or Passport)

Driver License or Florida ID

Auto Insurance & Registration
